Community Advisory Network (CAN)

Building stronger partnerships to better serve families across Alameda County.

The Community Advisory Network (CAN) is a group of trusted community leaders and service providers who guide Family Paths’ outreach efforts and ensure we stay connected to the real-time needs and resources within our region.

What the Community Advisory Network Does

Guides Outreach Strategy

CAN members provide valuable insight that helps shape our community engagement work. Their input ensures we meet families where they are—with messaging, materials, and support that reflect the diverse communities we serve.

Shares Program & Resource Updates

Members regularly share updates on events, programs, and services from their organizations. These timely exchanges allow Family Paths to make warm referrals and ensure families have access to a broad, up-to-date support network.

Fosters Collaboration

CAN creates space for mutual learning, cross-agency partnerships, and collaboration. Together, we identify gaps, share best practices, and co-create a more connected system of care for families.

Why It Matters

When community partners are informed and aligned, families receive better care. The Community Advisory Network (CAN) ensures that the web of support across Alameda County is strong, responsive, and connected. By sharing updates, surfacing gaps, and staying in conversation, CAN helps prevent families from falling through the cracks due to missed opportunities, duplicated efforts, or siloed systems.

The impact of this collaboration is more than theoretical—it’s reflected in the real stories of families who have been connected to the right support at the right time. Whether through a warm referral, a shared resource, or simply being seen and heard by a trusted provider, families benefit from a community of care that works together with purpose.
